Summary of Steps after Animal Registration

Here is what you need to know about how clients will locate your clinic information

(We recommend all staff review the program FAQ, also provided below.)

1. The client must first register through www.ACVO-SAVES.org website under the ‘Service Animal’ tab. They will receive a registration confirmation number which they are asked to bring to their appointment. You may reference this number however is best for your clinic. Registration helps ACVO monitor program volume and participating location handlings, information is only used to contact the client again the following year.

2. Once registered, the client will receive an email with a link to a participant list from which they can search for participating veterinary ophthalmology clinics by state. Once located, the client will call the clinic and make an appointment directly with staff. ACVO does not need any information regarding the appointments. If your appointment schedule fills up, please notify ACVO and we will mark it as such on our referral documents.

3. Prior to the in person appointment, it is advised that your clinic staff confirm that the client has ‘written proof of appropriate service/working/therapy animal training’ qualifying them for this program (this is not proof of anything more than the animal’s qualifications and has nothing to do with the medical situation of the individual client, that remains private). Paperwork can be provided from most service or working animal programs, such as completion certificates. Alternatively, for owner-trained animals a 3rd party validation, similar to the International Association of Assistance Dog Partners will work, demonstrating passing an assessment of some sort.

4. The client will see you for the screening exam. See FAQs but note that dilation is not required, this is optional, and some clients may wish to opt out if they are currently working. A simple exam form should be printed (below) and completed for the client. Your clinic may choose to keep a copy and give the client their own copy to provide to their primary care veterinarian. These forms are not to be sent to the ACVO.

EXAMINATION DOCUMENTS

  • Screening Eye Exam Form

    • This form is encouraged for use in the clinic for the client to take home with them, and to provide to their referring veterinarian to assist with any watch items or treatments about which their general veterinarians should be aware. This form does not need to be submitted to ACVO, and is not part of the ACVO CAER screenings through OFA.

  • Photography/Media Release Form  

    • If you choose to take photos for use in your clinic, social media, or to provide to the ACVO for promotion, please ask the client for permission and ask them to sign this form for use. The use could be by the clinic or ACVO for purposes of promoting the exam.
